Masseter Botox in Raleigh: when it is the right move and when it is not

TL;DR: Masseter Botox is a treatment that softens the contraction of the masseter muscle at the back of the jaw to decrease clenching/grinding and reduce lower-face width. It's the right call when the masseter is actually causing pain, or making the lower face appear wider, and the wrong call when the heaviness and width is coming from lost midface support, where softening the muscle can actually make the imbalance worse and increase the appearance of jowling.
Masseter Botox is one of the treatments patients ask me about most often, and it is also one of the treatments I think gets oversold the most.
For the right person, it can help soften lower-face heaviness, reduce the force behind clenching that causes pain, and leave the face looking more snatched and more like itself.

What is the masseter muscle and why does it affect facial appearance?
The masseter is the muscle on the side of your jaw that does the work of chewing. You can find it right now by clenching your teeth and pressing your fingers against the bulge that pops out along the back half of the jawline.
For some patients, that muscle is also part of a clenching or grinding pattern.
They may feel jaw soreness, morning tightness, headaches, tooth pressure, or tension that sits right along the lower face and can even extend into the neck. For these patients, the concern is not just cosmetic, it’s functional. The muscle is working harder than it needs to be, and the face can start to show the effects of that prolonger overativity over time.
A large or hyperactive masseter shows up in the mirror as a wider, more squared-off lower face which can cause the face to look heavier and more masculine. Treatment with masseter tox can not only be the key to reducing pain and tension in the lower face and neck, but help to slim and feminize the face.
How Botox and Dysport reduce masseter muscle activity
Botox, Dypsort, Xeomin and other neuromodulators can soften how strongly the muscle contracts. It does not reshape your facial structures unless the muscle is hyperactive and bulky.. The neuromodulator itself only acts to quiet the muscle.
The signal that normally travels from the nerve to the muscle gets temporarily blocked, so the muscle stops contracting as hard. Over a few weeks of working less the muscle softens and gets smaller creating the appearance of slimming of the lower face.
How long does masseter botox take to show results?
After about two weeks, you will start to feel the muscle soften. Most patients hit the peak effect somewhere between four and six weeks out. This is when slimming of the lower face begins to become visually apparent. The result typically holds for three to four months before the muscle starts regaining strength again.
The treatment works the way the body works, which is gradually.
Most patients come back every four to six months. Some find that after a year or two of consistent treatment, the muscle stays smaller and they are able to go longer between sessions because the hypertrophy pattern that built it up has been broken leading to longer lasting results and patient comfort.
When masseter Botox helps with TMJ pain and when it does not
Masseter Botox can soften the muscle that is contributing to TMJ, but if the issue is not mainly muscle-driven, Botox may not be the right answer by itself.
TMJ symptoms can involve the joint, the bite, the muscle, stress patterns, dental issues, and other factors outside the scope of a cosmetic injection appointment.
In your Aesthetic Consultation, I will ask you where you feel tension, when it happens, how often you clench, whether you wake up sore, and whether you have already been evaluated for bite or dental concerns.The goal is to get to the root of the cause of your pain, and find a solution that works for you specifically, be that with Botox injections, or referral to a specialist to ensure you get the right treatment for you.

Why a wide lower face is not always caused by the masseter muscle
If the masseter is not the reason the lower face looks wide, Botox may soften the muscle but still miss the reason the face looks unbalanced.
A jaw can look heavier because the muscle is overactive, but it can also look heavier because the midface is not supporting the face the way it used to.
My goal is to figure out which one is actually driving what you see, because the answer changes everything about whether masseter Botox is the right tool. I need to understand what is creating the concern before I can recommend anything, because the right result does not come from treating one feature at a time. It comes from choosing the treatment, timing, and sequence that make the most sense for your face.
Almost everyone who comes in is noticing something real. Jaw tension, lower-face heaviness, loss of support, volume changing in places that did not used to bother them, or just the sense that something is sitting differently than it used to.
There is almost always something we can improve. The part I care about is making sure we treat the real thing, not the thing it looks like at first glance.
When facial balancing is the better first step before masseter Botox
Sometimes the lower face is heavy because the upper and midface has lost support and volume as a natural part of the aging process, not because the masseter is doing too much.
The midface is where a lot of facial structure lives, and when that structure starts to change, the lower face can read as wider and heavier even though the masseter has not changed. In that case, softening the masseter does not solve the problem you came in for. It can actually make the imbalance more visible by decreasing the structural support it can provide to hold back some of the soft tissues and skin along the jaw.
When that is what I see in the assessment, the better starting point is usually rebuilding the structural support that has been lost. That might mean artfully and intentionally placed dermal filler to restore structure in the midface, or biostimulators like Sculptra or Hyperdilute Radiesse to help your body produce its own collagen and elastin over time.
Once the support is back, the lower face often looks different on its own, because what was creating the heaviness was never really about the masseter it was about the loss of structural support, descent and deflation of fat pads, and loosening of ligaments that happen during the facial aging process.
I rarely treat one feature in isolation

Proper dosing is based on the size and strength of each individual muscle. Masseter Botox is priced per unit, and the FDA dosing guideline recommends 20-40 units per side, but your dose is based on your unique anatomy.. The actual range depends on how strong your muscle is, what we are trying to achieve (pain relief vs facial slimming), and how it responds to the first treatment. How long do masseter Botox results last?
Masseter Botox results typically last three to four months. The muscle starts softening around two weeks after treatment, hits peak effect between four and six weeks, and gradually returns to full strength by the end of the three-to-four month window. Is masseter Botox better than jawline filler?
We can’t really compare the two becausehey do different things. Masseter Botox softens muscle that may be making the lower face look wider, while jawline filler adds structural volume to a jaw that needs more definition along the bone.
